Ibbott&Co Case Study: Repairing a chipped front tooth - AFTER

Using natural-looking composite bonding to repair a chipped tooth

Many patients come to see us because they’ve chipped a front tooth and want it repaired in a way that looks completely natural.
They’re often concerned about avoiding unnecessary drilling, maintaining the health of their teeth, or achieving a result that doesn’t change the look of their teeth overall.

Natural-looking composite bonding can be an excellent solution to these concerns, but only when it’s carried out with attention to detail and used in the right situation. In this article, we’ll walk you through a recent case study to demonstrate how a subtle and natural-looking result can be achieved.

Firstly, let’s look at what composite bonding actually is.

What is composite bonding?

Composite bonding involves carefully adding a tooth-coloured resin material to the surface of a tooth to improve its shape or repair damage.
It is:

  • minimally invasive (often no drilling required)
  • additive rather than destructive
  • can often be completed in a single visit.

However, not all composite bonding is the same. The final result depends heavily on planning, material choice, and the level of detail in the finish.

When is composite bonding the right choice?

Composite bonding has many applications and is a very viable alternative to traditional ceramic materials. It can be used for:

  • chips or fractures
  • managing spacing or shape discrepancies
  • tooth wear
  • cosmetic improvements ranging from individual teeth to multiple teeth.

It is not always the best option for more complex cases involving bite issues or significant misalignment, where orthodontic treatment may be more appropriate.

How long does composite bonding last?

Composite bonding is a durable and long-lasting treatment.
In many cases, it can last 5–7 years or more before requiring significant maintenance. However, we also see well-maintained cases lasting 10 years or longer, particularly when the bite is stable and the bonding is carefully looked after.

Longevity depends on a number of factors, including:

  • bite forces and tooth position
  • oral hygiene
  • diet and habits (such as nail biting or chewing hard objects)
  • smoking
  • regular maintenance and polishing.

Over time, composite may require minor refinements, polishing, or occasional repair, all of which can usually be carried out conservatively.

Is composite bonding right for you?

Composite bonding can be an excellent option for the right case, but careful assessment is essential.
In some situations, treatments such as orthodontics or tooth whitening may provide a more stable or natural result and we often use all three of these together.
The key is choosing the approach that best preserves your natural teeth while achieving the outcome you want.

Case Study: repairing a chipped front tooth

The patient’s concern

This patient attended after being hit in the face with a hockey stick and breaking her front tooth. Importantly, she wanted a result that looked natural and in keeping with her existing teeth.

Clinical assessment

On examination, we found that:

  • there was a large fracture to the corner of her upper right central incisor;
  • the surrounding teeth were healthy;
  • the patient’s bite was stable, making this a suitable case for a conservative approach.

Because the rest of the tooth structure was intact, this was an ideal situation to avoid more invasive treatments.

Treatment options discussed

As with all cases, we discussed a range of options:

  • composite bonding to repair the chipped edge; or
  • using a porcelain veneer, which is a more invasive option involving tooth preparation.

Given the size of the defect and the patient’s preference to preserve her natural tooth, we decided that composite bonding was the most appropriate choice.

How the composite bonding treatment is carried out

Composite bonding is a highly technique-sensitive procedure. In this case, we first created a digital design of the ideal tooth shape, which was used to produce a custom guide. This helps ensure that the final result is balanced, symmetrical, and in keeping with the patient’s natural smile.

The restoration was then carried out with careful attention to detail. The steps of the restoration went as follows:

  • the tooth surface was very lightly refined where the edge was irregular;
  • a carefully selected composite material was layered to match the natural tooth, using multiple shades and subtle effects to replicate the depth, translucency, and character of the adjacent front tooth;
  • the shape was sculpted by hand;
  • the surface was polished to replicate natural enamel texture and light reflection.

The result:
